Pick up and fill out an application after Mass and submit at the tables after Mass. Alternatively, you can download the form below in .doc format to print, fill out and submit after Mass / mail or fill out in Word or OpenOffice and email to tjhb07@gmail.com. File is a ".doc" (Document) within ".zip" (compressed file) format. Please follow the directions below: On Windows XP
Save TJ_app.zip to your computer. Double click to open the ZIP file. The file TJ_App_2007.doc will be within the zip file. Click and drag the file out to your desktop. Double click the file to open it and fill in your application information. Click File->Save As and save the file as your name (bob_sir.doc) and attach it to an email addressed to tjhb07@gmail.com. On Mac OS X
Save TJ_App.zip to your computer. Double click to open or open with Stuffit Expander. Extract TJ_App_2007.doc file, fill out application and "Save as" your name as the file name (bob_sir.doc).
